The Gateshead LINk Hospital Discharge Working Group met this morning to discuss a range of issues. They included the ongoing progress of the Gateshead LINk Hospital Discharge Questionnaire which we are having a great response to after its distribution via the Gateshead Health NHS Trust Foundation membership magazine. Keep an eye out for updates on the findings on the Gateshead LINk wesbite and progress on how overiding issues identified during this process are being taken forward by Gateshead LINk. The group is also looking into how patients’ discharge is being handled from a local authority perspective and how patients are supported after discharge by Local Authority services.
